When assessing a child with suspected community-acquired pneumonia, the approach should begin with a careful clinical evaluation of the symptoms and disease severity. An important step is to identify the most likely etiology. In younger children, viral causes are common, whereas typical bacterial pneumonia is generally associated with high fever, abrupt onset, a more impaired general condition and focal findings on lung auscultation.
Atypical pneumonia occurs mainly in school-aged children and adolescents. It is characterized by gradual onset, a persistent dry cough, less pronounced fever, headache and myalgia. The most commonly implicated pathogen is Mycoplasma pneumoniae.
Despite these features, there is often considerable clinical overlap between viral and bacterial presentations.
Treatment
Treatment depends on disease severity and the most likely etiology. In children without criteria for hospital admission and with suspected typical bacterial pneumonia, high-dose oral amoxicillin remains the first-line treatment.
When atypical pneumonia is suspected, particularly in older children, azithromycin may be considered. Treatment should be complemented by general supportive measures, including fever control, adequate hydration and clinical monitoring.
Caregivers should be informed that clinical improvement may not be immediate and that reassessment should be considered if the child deteriorates or fails to respond to treatment.
Complications
The main complications include pleural effusion, empyema, necrotizing pneumonia and respiratory failure. A complication should be suspected in the presence of persistent fever, significant chest pain, markedly reduced breath sounds or failure to improve clinically after 48–72 hours.
Early recognition of these conditions allows prompt adjustment of the treatment approach.
